If you would like to obtain a copy of an Illinois Traffic Crash Reconstruction Report, submit your written request, a $20.00 check or money order (payable to Illinois State Police), and a self-addressed, stamped envelope to the following address:

Your request should include the following information:

• Agency Crash Report Number
• Date of the crash
• Location of the crash
• Names of involved parties (drivers and passengers)

To obtain the report number or check the status of a Traffic Crash Reconstruction Report, please contact ISP.Patrol.Records.Unit@Illinois.gov . All requests for Illinois Traffic Crash Reports are subject to FOIA and you will be receiving a redacted version of the requested records.
